This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

The Privacy Policy applies to information and health care records maintained by the UCSD Division of Medical Genetics (DMG).

 

Your Medical Information

When a specimen is submitted from you for genetic/oncology/parentage testing, DMG creates a paper and electronic record of your medical information and the services received for use in your care and treatment. Examples of medical information include symptoms, diagnoses, treatment, tests ordered, test results, and interpretation.

 

Our Responsibilities

DMG will not use or disclose your medical information without your authorization except as described in this notice. DMG is committed to:

  • the protection and confidentiality of your medical information
  • making this notice available to you which describes our responsibilities and privacy practices with respect to your medical information
  • following the terms of this notice.

 

Use And Disclosure Of Your Medical Information

DMG abides by all applicable state and federal laws related to protection of this information. The following sections provide examples of use and disclosure of your medical information.

 

Treatment; DMG will provide your physician/health care provider/genetic counselor with test results which may be used to provide medical treatment or services.

Payment; DMG may use and disclose your medical information so that services provided may be billed to and payment received from you, an insurance company, or a third party.

 

Laboratory Operations; members of the UCSD medical staff, risk management, quality improvement programs, or credentialing agencies may use information in your medical record to examine laboratory testing policies and procedures. This information will then be used to improve the quality of laboratory services.

 

Appointment Reminders and Alternatives; DMG may contact you regarding an upcoming appointment. DMG may also provide you with information on testing alternatives or other services that may be of interest to you.

 

Communication with Family Members; DMG may disclose your medical information to anyone involved in your care including relatives, personal representatives, or any individual you identify.

 

Research; DMG is part of the University of California San Diego. UCSD is a research institution. DMG may use and disclose your medical information for research purposes provided these activities have been approved through a special review process to protect your safety, welfare, and confidentiality. Disclosure of your medical information for research purposes is subject to the confidentiality provisions of state and federal law.

 

Law Enforcement; DMG will disclose medical information about you when required by state or federal law or in response to a valid subpoena.

 

Correctional Facility; DMG may disclose medical information about you to a correctional facility as required by law.

 

Your Rights Regarding Medical Information About You

Regarding your medical information, you have the right to:

  • inspect and receive a copy of your medical information. Requests for a copy of your medical information must be made in writing
  • request changes to your medical information. Requests for changes to your medical information must be made in writing
  • inspect and receive a list of accounting disclosures of your medical information. Requests for accounting disclosures must be made in writing
  • request a restriction or limitation on your medical information that is disclosed for treatment or payment. Requests for restriction must be made in writing
  • request confidential communications of your medical information by alternative means. Requests for alternative communications must be made in writing
  • receive a copy of this Privacy Policy.
